The scene shows the real world, back in the cave.

Kabuto is fallen on the ground, out of his Sage Mode. His eyes are wide open and he is shaking heavily.
Sasuke: What did you do?
Itachi: Izanami...the power to bend the reality according to your immagination...the ability to affect your enemy's reality.*
Sasuke: What do you mean? He is in a genjutsu?
Itachi: No. He is not in a genjutsu. He is in an entire separate world. The world of his nightmares. Of his deepest fears. The Izanami brings the Hell on earth for the victim. His mind it's trapped in a separate dimension from ours. There, he is suffering the torments of Hell. He is under a great torture right at this point. Soon he will break and he will end the Edo Tensei. We didn't had enough power to break his body. But we have the power to break his mind. The psychological warfare it's the best for such an opponent.*
Sasuke: But at what costs, Itachi?
Itachi: Hmm...If i was alive i would lose the sight in both my eyes forever. But since i'm dead already...it won't affect me.
Sasuke: So what's the plan?
Itachi: We will wait until Kabuto breaks and releases the edo tensei. Unfortunately for him, the Izanami allows me to control his personal Hell. So i'm positive that he won't last that long.
Sasuke: It's enough.
Itachi: Indeed...so, brother...you wanted to talk with me. What do you want to know?
Sasuke: I'm just about to make a decision that will change the rest of my life.
Itachi: Getting revenge on Konoha or redemption?
Sasuke: How did you...!?!
Itachi: It doesn't matter, Sasuke. But what changed your mind? You were so full of hatred. What makes you think that you will be able to cast away all that hatred?
Sasuke: I don't know if i can...but...i have seen you how determined you were, and how much you regret the killing of our clan...so I have a question for you, Itachi...
Itachi: I'm all yours...ask me, brother...
Sasuke: It hurts? To kill all those innocent people?*
Itachi thinking: There is still hope...
Itachi: If it hurts? No, Sasuke it doesn't hurt...at first you don't realise exactly what you have done...but soon you start understanding that your life could have take a different path...and that's when the pain starts. It burns you from the inside, it consumes you, it makes you go insane. It makes you reconsider every decision that you ever took. It makes you think that your whole life was a mistake...and you will end up just like Kabuto...someone that lost completly sight of himself, that struggles to find a reason to live, to continue with this miserable life...killing the innocents in Konoha will haunt you for the rest of your life, Sasuke...
Sasuke makes a few steps back with a shocked expression. Suddenly his whole life passes in front of his life. He reminds his life with the clan and Itachi, the night when Itachi killed everybody, how he got into team 7 and their fight with Kakashi, how they took out Zabuza, the Chuunin exams, his fight with Naruto, killing Orochimaru, Itachi's death...-tears starts rolling down on his cheeks and the young Uchiha falls down on his knees crying. He grabs Itachi's legs and cries deeply.
Sasuke screaming: Forgive...forgive me Nii-san...i'm so sorry!! Forgive..me...!
Itachi grabs him by his right hand, pulls him up and looks at him straight in the eyes. Sasuke has a surprised face.
Itachi with a determined voice: There is always hope, Sasuke! Never forget that!*
